Transition Coordinator

 

The Transition Coordinator assists students with disabilities and their families in obtaining services

 

 

Essential Duties and Responsibilities:

 

  • Essential to build community relations with varying entities as it relates to student transition activities
  • B-13 Data Collection to ensure continued compliance with this State Performance Plan Indicator
  • Identify the connection between the student’s present level of academic and functional performance, post-secondary vision and transition activities including data collection on the implementation of these coordinated activities
  • Assessment of student employment abilities and interests
  • Assisting staff with the selection and implementation of transition assessments to use when developing the IEP
  • Providing guidance for districts so that they can support students course of study in order to reach their transition goals
  • Support the required educational development plan process and the desired use of personal curriculums
  • Provide technical assistance to the Special K portion of Section 380.1278b of the Revised School Code in regards to Personal Curriculums
  • Connecting providers, administrators, students and parents to agencies that will support a successful transition to adult life
  • Focus on Employment Outcome requirements for work based learning, coordinating experiences with student abilities and interests
  • Connecting to students to Career and Technical Education Centers and Post-Secondary
  • Education by coordinating activities to increase outcomes and participation for students with special needs
  • Attend IEP meetings upon request
  • Perform other related duties as assigned
  • Understanding of disability areas under IDEA.
  • Knowledge of how disabilities impact an individual’s ability to function in the post-secondary setting.
  • Knowledge of state and federal laws/regulations that protect individuals with disabilities in education, employment, and community.
  • Knowledge of adult service providers and services available within each.

 

Qualification Requirements

To perform this job successfully an individual must be able to perform each essential duty satisfactorily.  The requirements listed below are representative of the knowledge, skill, and/or ability required.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Education and/or Experience

  • A bachelor’s or graduate degree in special education or a field related to transition of youth with disabilities into adult life roles. Related fields include, but are not limited to, general and vocational education, vocational rehabilitation, and counseling.
  • A minimum of 3 years of satisfactory teaching experience in special or vocational education at the secondary level; or a minimum of 3 years of satisfactory employment providing transition-related service to individuals with disabilities between the ages of 13 to 26 years. Transition-related services include, but are not limited to, vocational rehabilitation, employment, counseling, independent living, and mental health. A person with a master’s degree in special education or field related to transition of youth with disabilities into adult life roles shall be credited with 1 year of employment.
  • Approval under competencies and procedures established by the department.

Employment Date, Salary and Work Year

Anticipated start date will be determined upon hire. Salary is determined as per the master contract.  Work days will be determined upon hire.
